DVLA Medicals
At YourGP we are experienced in offering the HGV, LGV, PCV (D4) licence medical examination.
- Appointments available at short notice, often same day
- Appointments available 5 days a week
If someone wears glasses for driving they will need to see an optician to complete the first part of the form before attending YourGP. At the appointment with us they will:
- Be asked to show current photo ID
- Complete a medical health assessment
- Receive a completed D4 form on the day

Are you a driver who needs to book their medical? Contact our team on reception@your.gp to arrange this appointment, but please note we will require your NHS GP records before the assessment takes place.
I was visiting with my son from California, US and he became ill. It was such a relief to find you and receive such expert care. He was diagnosed with bronchitis and we were given antibiotics right on the spot. My son was better in a few days and able to enjoy the remainder of our stay. Thank you very much.
