      Overcoming the anxiety of a smear test

      Going for regular smear tests is an effective way to protect yourself against cervical cancer. In fact, 75% of cervical cancers can be prevented by regular cervical screening. And whilst it’s not exactly a fun experience, for the majority of women it is just another routine appointment, over in a mere matter of minutes. For others, however, even the thought of a smear test can trigger anxiety. Here’s how the health experts at YourGP are helping to reduce the stress of smear tests.

      What happens during a smear test?

      During a routine smear test, a sterilised medical instrument, called a speculum, is inserted into the vagina. The speculum is gently expanded to hold the vaginal walls open so that the cervix is visible. A small, specially designed brush is then inserted to collect cells from the cervix. The whole procedure takes no more than a few minutes and although it may be a little uncomfortable, it shouldn’t cause any pain.

      If no high-risk HPV is found in the sample, you will not require another smear test for 5 years. This is because the risk of developing cervical cancer within 5 years is very low – it usually takes around 10 to 15 years for cancer to develop after an HPV infection.

      If high-risk HPV is found in the sample, the sample will then be used for cytology-based tests. Around 1 in 100 women will have precancerous changes highlighted in a cervical smear, which will require further treatment. Should any abnormal cells be detected, your doctor will inform you of the results and the recommended next steps.

      Common barriers to smear tests

      Despite the importance of these tests, the team here at YourGP understand that some women find them uncomfortable and for others even the thought of having a smear test can be triggering. There is a whole range of potential reasons for this:

      • Tilted cervix or uterus: This can sometimes make the cervix more difficult for the nurse or doctor to locate, so these appointments can sometimes take longer.
      • Endometriosis: This condition causes tissue to grow outside the uterus, making it more sensitive.
      • Menopause: The drop in oestrogen associated with the menopause can cause vaginal dryness which can make the procedure more uncomfortable.
      • Scarring: Scar tissue from a previous surgery can make the smear test more uncomfortable.
      • Post-traumatic stress disorder (PTSD): The procedure can be triggering for some people who have experienced previous medical or birth trauma.
      • Experience of sexual violence: Similarly, those who have experienced sexual assault may find even the thought of an intimate physical examination distressing.
      • Mental health conditions: For anyone with a condition such as anxiety, smear tests can seem daunting and overwhelming.

We find that giving patients time to sit, take a few deep breaths, and prepare for their smear test without feeling hurried can make all the difference to their experience, and is especially beneficial to those who feel anxious about the test.

      We are always happy to make adjustments and accommodations where possible. For example we can use a smaller speculum or have you try lying in a different position such as on your side.

      We are also happy for you to bring along a loved one to sit with you throughout the entire appointment if you wish.
